What Is Gestational Diabetes and Who Is at Risk?

Gestational diabetes mellitus (GDM) is a form of diabetes that develops during pregnancy in women who did not have diabetes beforehand. It occurs when the hormonal changes of pregnancy — particularly rising levels of human placental lactogen, cortisol, and oestrogen — cause insulin resistance, meaning the body's cells do not respond effectively to insulin and blood sugar rises above normal thresholds. Approximately 5–15% of pregnant women develop GDM, depending on the population studied and the diagnostic criteria used, making it one of the most common pregnancy complications worldwide.

Risk factors are multiple and interact with each other. Pre-pregnancy overweight or obesity (BMI ≥ 25) is the single strongest modifiable risk factor, associated with a 2–4-fold increased risk. Other significant factors include advanced maternal age (over 35), a previous pregnancy complicated by GDM, a family history of type 2 diabetes in a first-degree relative, polycystic ovary syndrome (PCOS), previous delivery of a baby weighing over 4.5 kg, and certain ethnic backgrounds including South Asian, Middle Eastern, East Asian, Indigenous, Pacific Islander, and Hispanic populations. Multiple pregnancies and excessive gestational weight gain also elevate risk. Importantly, some women with GDM have no identifiable risk factors, which is why universal or semi-universal screening is recommended in most countries.

Prevention: What You Can Do Before and During Pregnancy

The most evidence-backed preventive strategies target insulin resistance before and during pregnancy. Preconception weight management is the highest-impact intervention for overweight women: studies show that achieving even a 5% reduction in pre-pregnancy body weight can meaningfully reduce GDM risk. During pregnancy, excessive weight gain — defined by the Institute of Medicine as more than 11.5–16 kg for normal-weight women and more than 7–11.5 kg for overweight women — significantly increases the risk of GDM in the second and third trimesters. Regular obstetric monitoring of weight gain and early dietary counselling are therefore important preventive care components.

Physical activity is a powerful tool. A meta-analysis of over 30 randomised controlled trials found that women who exercised during pregnancy (150 minutes per week of moderate-intensity activity such as brisk walking, swimming, or prenatal yoga) had a 28% lower risk of developing GDM compared to sedentary controls. Exercise works by improving skeletal muscle insulin sensitivity and reducing adipose tissue inflammation. For women already diagnosed with GDM, post-meal walks of just 10–15 minutes have been shown to meaningfully blunt postprandial blood glucose spikes. Resistance training is equally effective: squats, lunges, and resistance band exercises performed 2–3 times weekly support glycaemic control without requiring gym access.

Dietary Strategies for Blood Sugar Control

Nutrition is the cornerstone of GDM management. The goal is not caloric restriction — pregnant women with GDM still need adequate energy and nutrients for fetal growth — but rather carbohydrate distribution and quality optimisation. Current evidence recommends distributing carbohydrate intake across 3 meals and 2–3 snacks daily to prevent large postprandial glucose excursions. Breakfast deserves particular attention: insulin resistance is highest in the morning due to the cortisol awakening response, making high-carbohydrate breakfasts particularly problematic. Many women with GDM find that switching to a protein-rich, low-carbohydrate breakfast (eggs, Greek yoghurt, nuts) dramatically improves fasting and morning blood glucose readings.

Carbohydrate quality is as important as quantity. Foods with a low glycaemic index (GI) — oats, legumes, whole grain bread, sweet potato, most vegetables, and most fruits — produce a slower, more gradual glucose rise compared to high-GI foods (white bread, white rice, sugary beverages, pastries). Pairing carbohydrates with protein, fat, and fibre further slows glucose absorption. Practical swaps include replacing white rice with cauliflower rice or brown rice, choosing sourdough bread over white bread, adding vinegar to meals (which reduces GI by delaying gastric emptying), and prioritising non-starchy vegetables at each meal. A referral to a registered dietitian specialising in GDM is one of the most valuable resources available and is strongly recommended.

Monitoring, Medication, and Medical Management

Most women with GDM are asked to monitor their blood glucose levels at home using a glucometer, typically measuring fasting glucose on waking and 1–2 hours after each meal. Target ranges vary slightly by country and guideline, but a widely used framework is: fasting below 5.3 mmol/L (95 mg/dL), one-hour post-meal below 7.8 mmol/L (140 mg/dL), and two-hour post-meal below 6.7 mmol/L (120 mg/dL). Continuous glucose monitoring (CGM) devices are increasingly being used in GDM and offer valuable real-time data, helping women and their care teams understand individual glycaemic responses to specific foods and activities.

If diet and exercise fail to achieve target glucose levels within 1–2 weeks, pharmacological treatment is recommended. Metformin is used in many countries as a first-line oral agent; it improves insulin sensitivity, is considered safe in pregnancy, and avoids the hypoglycaemia risk of insulin. However, some studies suggest that up to 50% of women started on metformin will ultimately require insulin supplementation. Insulin therapy — administered via subcutaneous injection, typically basal insulin at night and short-acting insulin before meals — is the most effective and flexible option, with decades of evidence for safety in pregnancy. All pharmacological decisions are made with and by your obstetrician or endocrinologist based on your individual glucose patterns and response to lifestyle modification.

Impact on Your Baby and Long-Term Health

When maternal blood glucose is consistently above normal, excess glucose crosses the placenta to the fetus, stimulating the fetal pancreas to produce more insulin. Fetal insulin acts as a growth hormone, causing macrosomia — babies born significantly larger than average. Macrosomia increases the risk of shoulder dystocia (where the baby's shoulder gets stuck during delivery), birth trauma, and emergency caesarean. Babies of mothers with poorly controlled GDM are also at higher risk of neonatal hypoglycaemia (low blood sugar at birth, requiring monitoring and sometimes glucose supplementation), jaundice, and respiratory distress syndrome. The good news is that with well-controlled GDM, these risks are substantially reduced and birth outcomes approach those of the general population.

The long-term implications extend beyond the immediate postpartum period. Research consistently shows that children born to mothers with GDM have an elevated lifetime risk of obesity, impaired glucose tolerance, and type 2 diabetes — a phenomenon sometimes described as "metabolic programming" in the womb. For the mother, having GDM is one of the strongest predictors of future type 2 diabetes: approximately 50% of women with a GDM history will develop type 2 diabetes within 10 years. This makes postpartum follow-up non-negotiable: a 75 g OGTT at 6–12 weeks after birth and annual fasting glucose or HbA1c checks thereafter. Sustained breastfeeding reduces both maternal and infant risk. For both mother and child, the gestational diabetes diagnosis is an opportunity to establish lifestyle patterns that protect health across the lifespan.

How is gestational diabetes diagnosed?

Gestational diabetes is typically diagnosed through an oral glucose tolerance test (OGTT) between 24 and 28 weeks of pregnancy. You drink a glucose solution, and blood is drawn at 1 and 2 hours to measure how your body processes sugar. Women at higher risk (previous gestational diabetes, BMI over 30, family history of type 2 diabetes, or certain ethnic backgrounds) are often screened earlier in the first trimester.

Does gestational diabetes harm my baby?

Unmanaged gestational diabetes increases the risk of macrosomia (large baby), birth injuries, neonatal hypoglycaemia, premature birth, and a higher lifetime risk of obesity and type 2 diabetes in the child. However, with diligent blood sugar management through diet, exercise, and if necessary medication or insulin, outcomes for both mother and baby are generally excellent and comparable to non-diabetic pregnancies.

Will gestational diabetes go away after delivery?

For most women — approximately 90% — blood glucose levels return to normal within 6 weeks of delivery. However, having gestational diabetes significantly increases your lifetime risk of developing type 2 diabetes: approximately 50% of affected women will develop it within 5–10 years.
