Understanding Preeclampsia

Preeclampsia is one of the most serious pregnancy complications, affecting 2–8% of all pregnancies worldwide. It is characterized by high blood pressure (hypertension) and signs of damage to organ systems, most commonly the liver and kidneys, developing after 20 weeks of pregnancy. The exact cause is not fully understood, but it is believed to involve abnormal placentation — where the blood vessels supplying the placenta do not develop properly, leading to reduced blood flow and widespread vascular changes in the mother. Left undetected and untreated, preeclampsia can progress to eclampsia (seizures), HELLP syndrome, stroke, and can be life-threatening for both mother and baby.

Risk factors for preeclampsia include a first pregnancy, a previous history of preeclampsia, carrying multiples, pre-existing hypertension or kidney disease, obesity, diabetes, autoimmune conditions like lupus, and a family history of the condition. Women with multiple risk factors may be offered low-dose aspirin from 12 weeks of pregnancy as a preventive measure — this has good evidence behind it. Regular blood pressure monitoring and urine protein testing at prenatal appointments are the front-line tools for early detection.

Gestational Diabetes: More Than Blood Sugar

Gestational diabetes mellitus (GDM) occurs when the body cannot produce enough insulin to overcome the natural insulin resistance of pregnancy, causing elevated blood glucose levels. It affects approximately 6–9% of pregnancies in developed countries, with higher rates in women from certain ethnic backgrounds. GDM is associated with a range of complications for both mother and baby: for the mother, increased risk of caesarean delivery, high blood pressure, and future type 2 diabetes; for the baby, excessive fetal growth (macrosomia), birth injury, low blood sugar at birth, and a higher long-term risk of obesity and metabolic syndrome.

The encouraging news is that gestational diabetes is highly manageable. The vast majority of cases are controlled through dietary modification and regular physical activity alone. Around 10–20% of women with GDM will need medication (metformin or insulin) to achieve adequate glucose control. Close monitoring through blood glucose testing, regular growth scans, and increased prenatal contact means that with appropriate management, outcomes for both mother and baby are generally excellent.

Placental Complications: Previa and Abruption

Placenta previa and placental abruption are two distinct but serious placental complications. Placenta previa — where the placenta covers the cervix — typically causes painless, bright red vaginal bleeding, usually in the second or third trimester. It is often detected on routine ultrasound before symptoms appear. Women with complete placenta previa are advised to avoid sexual intercourse and vaginal examinations, and will deliver by planned caesarean section. Placental abruption, by contrast, involves the placenta separating from the uterine wall before delivery, causing painful bleeding and potentially cutting off oxygen to the baby. It is a medical emergency requiring immediate intervention.

Risk factors for placental abruption include previous abruption (the strongest single risk factor, with a recurrence rate of 5–17%), high blood pressure, smoking, cocaine use, trauma to the abdomen, and rapid reduction of uterine volume (as can occur after rupture of membranes in a twin pregnancy). The management depends on gestational age, the severity of the abruption, and the condition of both mother and baby — ranging from careful monitoring and bed rest to emergency caesarean delivery.

Preterm Labor: Recognition and Response

Preterm birth — delivery before 37 completed weeks — occurs in around 10% of pregnancies globally and is the leading cause of death and disability in newborns. The signs of preterm labor include regular, painful uterine contractions (four or more in an hour), persistent low back pain that does not respond to position changes, pelvic pressure as though the baby is pushing down, watery or bloody vaginal discharge, and a sensation that something is different or wrong. Not all of these signs mean that preterm labor is actually occurring — many women who present with these symptoms do not deliver preterm — but all warrant immediate assessment.

If preterm labor is confirmed before 34 weeks, corticosteroid injections (to mature the baby's lungs), magnesium sulfate (to protect the baby's brain), and tocolytics (to try to delay delivery) may all be offered, depending on the clinical picture. The goal is usually to reach a hospital with appropriate neonatal intensive care facilities and, if possible, to gain enough time for corticosteroids to take effect. Women with a history of preterm birth or with a short cervix detected on scan may be offered preventive treatments such as progesterone supplementation or a cervical cerclage (stitch).

When to Seek Emergency Help

Understanding the threshold for seeking urgent care during pregnancy can save lives. You should contact your maternity unit or go to the emergency department immediately if you experience: heavy vaginal bleeding at any stage of pregnancy; a severe, persistent headache combined with visual disturbances or upper abdominal pain; sudden severe swelling of the face, hands, or feet; reduced or absent fetal movements after 28 weeks (use your hospital's formal fetal movement counting guidance); chest pain or difficulty breathing; a seizure or loss of consciousness; signs of infection such as high fever, chills, and lower abdominal pain; or any symptom that feels significantly different or "wrong."

Trust your instincts. Research consistently shows that women who act on a gut feeling that something is wrong during pregnancy often have that concern validated. Healthcare systems are designed to assess, not to dismiss, and no maternity unit will fault you for attending with a concern that turns out to be benign. The risk of not seeking assessment when a serious complication is developing is far greater than the inconvenience of a precautionary check. What are the warning signs of preeclampsia?

Preeclampsia warning signs include sudden swelling of the face, hands, or feet, a persistent severe headache that does not respond to paracetamol, visual disturbances such as blurred vision or seeing flashing lights, pain in the upper right abdomen (below the ribs), and nausea or vomiting in the second half of pregnancy. Most cases resolve after delivery, but women with gestational diabetes have a significantly elevated lifetime risk of developing type 2 diabetes.

What is placenta previa and is it always dangerous?

Placenta previa occurs when the placenta partially or completely covers the cervix, obstructing the birth canal. It is found in roughly 1 in 200 pregnancies at term. It can cause painless vaginal bleeding, often appearing in the second or third trimester. Minor (marginal) placenta previa may resolve as the uterus grows and the placenta moves upward. Complete placenta previa, where the placenta fully covers the cervix, requires planned caesarean delivery and carries higher risks of significant bleeding. Any vaginal bleeding in pregnancy should be assessed promptly by a healthcare professional.

Can pregnancy complications affect future pregnancies?

Yes, several pregnancy complications increase the risk of recurrence in future pregnancies. Women who have had preeclampsia have a 16% risk in a subsequent pregnancy (higher if it was severe or early-onset). Gestational diabetes has a recurrence rate of around 40-50% in the next pregnancy. Preterm birth also has a significant recurrence risk. However, early monitoring, prophylactic low-dose aspirin (for preeclampsia risk reduction), careful blood glucose management, and cervical monitoring can all reduce risks in future pregnancies.
